Why my hair breaks off when I comb?

Hi, this is a question that has me very concern. Well first of all I am 49 years of age, my hair has always been very thin and it has constantly broken off when ever I comb my hair, Know I have a problem and I really don't know what is going on, because when ever I comb or touch my hair or even touch my scalp it really hurts alot. Could you please tell me whats going on and should I be concern? also could you tell me what type of shampoo and conditioner I should use.
